That I am entitled to get the Service Connection to the premises as per provision of the Indian Electricity Act 2003, on observing all formalities of law and on payment of all charges for obtaining such service connection.

3. That I am ready to comply with all the directions of the WBSEB as per terms and condition of the Agreement and Provisions of the General Conditions of Supply.

There is no outstanding dues against the portion of the premises where new connection is prayed for.

No service connection was / is existing in the portion of premises where new connection is being applied.

Low and Medium Voltage mains is available in front of my house and the distance from nearest pole to my service bracket will neither require further support nor cross plot of others (no objection of the owner whose plot may cross is enclosed) nor exceeds 30 meters in length.

My premises is solemnly occupied by me/or a unit of a multiownership complex (attested copy of receipt of holding tax/ ownership deed/ agreement is enclosed).

The Electric Connection to be given by WBSEB in my premises does not establish any right or claim over the land in question as stated.

That the WBSEB will not incur any liability in case of my premises is demolished by any competent authority for greater interest of public or for their departmental interest and I shall inform the WBSEB about such demolition before hand so that Board can remove their equipment and I shall be bound to pay any payments or dues as to be claimed by Board.

That I am swearing this Affidavit in relation to the Agreement as agreed in the WBSEB prescribed form (WBSEB- 11) in all respect to the provision of Indian Electricity Act 2003 and the rules for the time being in force may also be applicable.

That if any of the above stipulations is violated by me WBSEB may not bound to effect service connection in spite of deposition of charges and compliances of all formalities.
